I tried going to the App IDs and Provisioning Profiles but still the same and do not know how to fix it.
Such errors happen when the certificate and provision profile do not match. The simplest solution is to delete all keys, revoke the certificates in Apple console and create new ones: Creating a certificate and provisioning profile

Yes. You need to test it with a different device. iPhone 4 (not S) is a very old device now. I wouldn't use it for testing. At some point iOS 8 will be the minimum version. You can see Apple statistics here: https://developer.apple.com/support/app-store/
