Icons

Hello,
I've just finished my first application and now I want to add an icon to the exe.

Can you tell me, please, what is the preferred format for the icon (dimensions, number of colors etc) so I can design or find a good one, please?

Icons should have 256 colors.
You can have a 16*16 and a 32*32 pixel icon in the file.
