I believe they use the Major version number hence 15, so sometimes they may release security update or patch and they will update the minor version number.

I have iPhone 5s which is also stack on Version 12 that is 12.5.7

Can I use it without hosted builder or Mac just to test b4i code?
I have an Apple developer account and I also have an iPad and iPhone 7.

No. Developer account is not enough

on Windows you need Hosted Builder with iPhone as simulator
or
MacBook with XCode installed which you can create simulator on or with iphone
