We began the review of your app but are not able to continue because we need access to a video that demonstrates your app in use on an iOS

Since when a video is required?

I think they could not figure out what to do with the app so they want a video. You dont have to provide a video just describe them what to do detailed.

They also reject my app (metadata reject) because Google AdMob Test Banner is shown on screenshot and review waiting time is about 11 days.

I wonder who are in that app review team and how they are selected (probably the cheapest unemployed people they could find).

The screenshot tells it all, you don't even need to install that app to know what it's for.
You even know by just looking at the appname.
